Established in 2018, Wellness Five is a boutique health and wellness company founded by Lisa Burbage, a National Board Certified Health & Wellness Coach. Lisa and her team help individuals make meaningful, sustainable changes to improve their physical, mental, and emotional well-being.

At the heart of Wellness Five is our evidence-based prevention work. We are a fully recognized provider of the CDC’s National Diabetes Prevention Program and a Medicare-enrolled supplier of the Medicare Diabetes Prevention Program.

Lisa also serves as a CDC Master Trainer and DTTAC Community Coach Specialist, helping train, mentor, and support Lifestyle Coaches delivering the diabetes prevention programs nationwide.

From year-long lifestyle change programs to workshops and speaking engagements, Wellness Five makes better health feel practical and achievable. Our approach focuses on realistic strategies, supportive coaching, and small changes people can sustain in real life.

Meet Lisa: The Story Behind Wellness Five

Lisa Burbage knows firsthand that lasting change rarely happens overnight.

After overcoming food addiction and breast cancer, Lisa left a successful career in real estate with a new sense of purpose: to help others take better care of their health before a crisis forces them to.

Today, Lisa is a National Board Certified Health & Wellness Coach, founder of Wellness Five, and a nationally recognized leader in lifestyle change and diabetes prevention. Since 2018, she and her team have helped thousands of people make meaningful, sustainable changes to their health and well-being.

Lisa believes wellness shouldn’t require perfection—or an overhaul of your life. Through practical tools, evidence-based strategies, and coaching support, Wellness Five helps people turn what they know they should do into small, realistic actions they can actually sustain.

Whether she is leading a group program, speaking to an organization, or training and supporting other coaches, Lisa’s goal is the same: to help people build the habits, confidence, and support they need to feel better, stay healthier, and keep doing the things they love.
